A 10-year study, recently published in the journal Obesity Surgery, offers important insights into the long-term outcomes of laparoscopic sleeve gastrectomy. The findings provide reassuring evidence that this bariatric treatment can deliver lasting benefits, not only in terms of weight loss but also overall well-being and patient satisfaction.

What the Study Looked At
The study followed 300 adults with severe obesity who underwent a primary sleeve gastrectomy, with 193 patients completing the full 10-year follow-up.
All participants met established criteria for bariatric surgery, including a body mass index (BMI) of 40 or above, or 35 and above with obesity-related conditions such as type 2 diabetes, hypertension, or high cholesterol.
Over the 10-year period, researchers assessed a range of outcomes, including weight and BMI changes, obesity-related health conditions, physical activity and lifestyle habits, patient-reported quality of life, and overall satisfaction with surgery.
This long-term data is particularly valuable, as much of the existing evidence around gastric sleeve surgery focuses on shorter timeframes of one to five years.
Sustained Weight Loss Over Time
The study showed that sleeve gastrectomy leads to significant weight loss, especially within the first year after surgery. On average, patients saw their BMI drop from 42.5 to 29.1 within 12 months. This level of weight loss can have a positive impact on overall health and help reduce risks linked to obesity.
Over time, some weight regain is normal. From around the third year onwards, patients experienced a gradual increase in weight, with average BMI rising to 31.9 at the 10-year mark. However, this does not mean the surgery has failed.
Importantly, more than half of patients maintained over 25% of their excess weight loss even after 10 years. This shows that while sleeve gastrectomy is not a quick fix, it can be an effective long-term solution when combined with healthy lifestyle changes.
Improvements in Quality of Life
Beyond weight loss, one of the most important findings was the improvement in quality of life. After 10 years, 86% of patients reported better overall well-being, with more than half describing their results as excellent.
The greatest improvements were seen in self-esteem, physical activity, and social life. Many patients felt more confident, more active, and better able to engage in everyday activities.
Most patients continued to experience lasting positive changes in their quality of life compared to before surgery. This highlights that bariatric surgery can deliver meaningful, long-term benefits that go well beyond weight loss.
The Role of Lifestyle and Behaviour
The study also highlighted an important point: long-term success is closely linked to lifestyle habits.
At the 10-year follow-up, around 60% of patients were taking part in regular physical activity, while a significant number remained inactive. This helps explain why some people maintain better results than others over time.
Sleeve gastrectomy is a highly effective procedure, but it works best when combined with ongoing support, healthy eating, and regular activity. Patients who stay engaged with follow-up care and build healthier routines are more likely to maintain their weight loss and overall health improvements.
High Levels of Patient Satisfaction
Despite some weight regain over time, most patients remained happy with their results. More than 85% reported being satisfied with their surgery, with many describing their outcomes as very good or excellent.
Only a small number of patients felt they were worse off than before surgery, usually because they did not lose as much weight as expected. This highlights the importance of having realistic expectations and receiving the right guidance throughout the journey.
